FridgeFreezer
Just found this story on the BBC news website - seems up to 130,000 blank V5's have gone missing, which unsurprisingly has made life quite easy for those cloning cars. Buyers are advised to check the serial number of the V5C before buying. I suppose it makes a change from losing secure data

Godlykepower
This was on the news earlier. They printed them in 2006 and straight away realised something was wrong, so sent them to be shredded and "somehow they never were shredded" according to the Chief Exec of the DVLA. Bless 'em. hobson
i'll bet the person responsible has probably been promoted! and the reason will be something like: 'the team memeber noticed the glaring error on their part and acting in accordance with government guidelines reported the error thereby alerting us to this lapse in procedure, but from this lessons will be learned' or some such carp!
